Epigastric pain: Most likely gastritis and or H pylori ( a bacterial causing gastritis) or gallbladder orrarely if acute a pulmonary embolism especially if smoking and on BCP or asthma/ bronchitis/ penumonia. Seek medical attention immediately in ER if there is shortness of breath
