Cilnidipine: Hi. I assume your Rancil is cilnidipine, in which case amlodipine is a mechanistic substitute. However, I would use an ACE inhibitor or angiotensin receptor blocker first line, and only use a dihydropyridine calcium channel blocker as an add-on. Do you know if your urinary albumin:creatine ratio is elevated? With your creatinine, I'm guessing so. Talk with your doc about an ACEi or ARB. Good luck!
