Maybe: I think some "tension" headaches are actually mild or atypical migraines or some kind of mixed headache syndrome; & there are people whose brains don't read the "official" headache classification and whose headaches don't quite fit any category. The terminology is intended to promote consistency of usage among clinicians, not to be perfect & cover everyone. What matters is what treatments work.

Of course: Tension Type Headache may exist without any evidence of pericranial muscle tenderness. It is not unusual for patients with TTH to experience migraine-like symptoms (throbbing headache, visual disturbances, nausea). See your PCP for evaluation and treatment.
