Astigmatism?: An eye exam is definitely in order. Astigmatism can cause the symptoms you described as can eye muscle fatigue. You may need corrective lenses or to take more frequent breaks from the computer.

Migraine: If this problem only occurs some of the time, and you are able to focus your eyes at other times, this could be a symptom or aura of migraine.
