See below: Discharge stains in the underwear can be from her vagina, urethra (where the urine comes out) or her recturm (where stool comes out). Can you tell where it is coming from? Does she have pain or itching? Some possible causes are poor hygiene, leaking of stool (if in rectal area), leakage of urine (possible urinary tract infection) & other. I would advise seeing her doctor to help you figure it out
