A few things: Depending on how much excess skin, and how much fat there is to deal with, a brachioplasty (or upper arm lift) can vary. The basic procedure for someone with minimal skin sag and some extra fat would be liposuction alone. For more skin excess we would take a small amount of skin from the armpit only, but for a large amount of skin, a longer incision would be necessary. See a good plastic surgeon.

Arm Lift Surgery: Brachioplasty involves removing excess, hanging skin from the upper arms. Sometimes this skin extends into the armpit and so the incision can extend into there and along the lateral chest wall in an "l" shape. Liposuction of the area to be excised can be a helpful adjunct to excision. In the end, you are trading in "bat wings" for a scar and so you have to weigh this tradeoff carefully.

Excess Skin Arms : Arm lifting surgery is a very patient pleasing operation for the appropriately selected patient. This operation removes excess skin/adipose tissue of the upper arms. Careful planning is important to help place the resulting scar in as favorable a position as possible. I like to place the resulting scar along the lower border of the arm; hopefully not seen from the front or back of you. Best.

It Depends: Brachioplasty, which reduces excess skin and underlying fat between the underarm and the elbow, can be performed at varying degrees, depending on the specific case. The surgical plan is generally based on the length of your arm and the amount of hanging skin. http://www.cosmeticsurg.net/procedures/arm-lift/
