May be an infection: You might have a septic joint hence the lack of response to your usual meds. You are on immunosuppressive medications so at greater risk for infection. Suggest you have your doctor examine your knee as soon as possible.

See ortho doc: Unless you have a fever, and a hot swollen joint that does not allow any motion (signs of an infection) it is possible that your arthritic knee is in need of a knee replacement. Get evaluated by an orthopedic doctor
