Not necessarily: Fever can be expected for the first 72hrs after starting antibiotics. If acting okay, and staying hydrated then it's okay to monitor. If temp remains high 90 minutes after Motrin dose then a dose of acetaminophen can be given. If fever persists outside that 72 hrs then it warrants another look at the peds office. Make sure antibiotics are given as prescribed. Go to ED if confusion, or higher temp

Not just yet: Sometimes it may take antibiotics 48 hrs to kick in and it is not unusual to still have a fever for up to 48 hrs before pt improves. If fever persists for more than 48 hrs then you might be dealing with drug resistant bacteria. For the fever make sure you are dosing motrin at 10 mg/kg and can give every 6 hrs. If pt is not vomiting, not lethargic, doing well when no fever, then be patient, wait
