Some ideas: Does he have aortic regurgitation? Does somebody have an idiosyncratic way of taking blood pressure, or imagine that an accurate diastolic pressure is the cessation of ALL sound, and perhaps he has a vascular sound unrelated to the Korotkoff sounds? I'll bet the latter.
