Not necessarily: From the American Academy of Pediatric Dentistry: If a child presents with severe symptoms of pulpitis, treatment (i.e., pulpotomy, pulpectomy, or extraction) should be rendered. Antibiotic therapy usually is not indicated if the dental infection is contained within the pulpal tissue or the immediately surrounding tissue.

Unusual: Unusual for a pulpectomy of and by itself to be done on a permanent tooth. Are you seeing an Endodontic Specialist?

Depends: antibiotics are to be used only if there is infection.In many root canals, pulp tissue is inflamed,not infected. The doctor performing the procedure should know the difference and prescribe accordingly. Even in infected cases antibiotics are not always needed.
