Appendectomy 1 month ago and i had constipation 3 days ago which made the pain re appear again in the incision area could it be a hernia ?

Unlikely: Most likely the straining associated with constipation has pulled at some of the internal sutures and mildly reinjured the healing tissues around the appendectomy incision. Unlikely this would have been enough to cause a hernia, but time will tell. Nothing to do but observe, and take measures to keep stool soft: drink fluid, eat fiber, mild exercise:)
