Possibly: For the most part the tests we have are pretty good at finding gallstones in the gallbladder and main bile duct. If stones are being formed higher up in the liver they may go undetected with ultrasound and CT scan. Most likely the bilirubin will be elevated in this scenario.

Depends on the tests: If you undergone and abdominal ultrasound, CT scan and MRCP(a special liver/hepatobiliary protocol MRI), and nothing was seen, it's quite unlikely that a stone was missed. And if you've subsequently undergone an ERCP with a cholangiogram(dye injected into the biliary tree) and nothing was found even less likely.
