I'm a otherwise health 22 y-old with gallbladder sludge that is inflamed, severe pain after fatty meals. is diet the best way to treat it?

No, surgery might be: If your gallbladder is truly inflamed and symptomatic, it has to be removed surgically to effect symptom relief. Consult with a general surgeon.

Probably not: If you have signs of GB irritation (I'm assuming on an ultrasound) and you are having consistent, severe pain then diet modification probably won't be enough to stop the pain. You can try it, but in the end you will probably require surgery to remove the GB (laparoscopic cholecystectomy). This routine procedure is safe and effective, and commonly performed in the US.
