Insurance rip off: We have relegated health, our most intimate aspect of our lives, to a monolithic, impersonal, bureaucratic entity whose way of surviving and making a profit is by withholding treatment. So when you have patients who are depress, psychotic, can't think straight and embarrassed, it is much easier to deny treatment, approve a couple of visits at a time, reject medications, labs or other treatments.

Reimbursement: The rates of reimbursement are often quite low, and so psychiatrists and psychopharmacologists (and many other mental health practitioners quite frankly) opt out of panels. Many of these panels pay still what they were paying years ago, and relying on many panels is not conducive to making a living. Also, insurance companies have more say about how, who, and when patients will be treated.
