No, not common: Billing for more time than was actually spent with a patient is not common. Sometimes sessions have to be billed as "extended" depending on complexity, though. Still, 3 extra hours for each sounds excessive unless additional testing, etc was done. Seems like if done, that should have been noted as such in the billing. Your son needs to ask about his bill to correct any errors.

Not okay: The mental health professional should bill the insurance company for the actual service s/he provides. Possibly there was a mistake or miscommunication somewhere, or a billing error. If the doctor is deliberately billing for services s/he did not actually provide, that would be insurance fraud.
