Anal fissure: Pain on defecation is often due to an anal fissure, which is a split in the skin of the anal canal. The cause of the fissure can be passage of very hard stool, but in many patients the cause is unclear. Initial treatment may be sitz baths, high fiber diet, stool softener and topical medication for pain relief. If pain persists see a physician for proper diagnosis and treatment.
