Usually two: One, if the implant is unfortunately not implanted correctly so that the hormones aren't released/absorbed by your body in the expected fashion. Two, is just that, no birth control, even if used 100% correctly has a 0% failure rate, in that your body sometimes "has a mind of its own" and will still ovulate through hormonal manipulation!
