Complicated: You are describing a situation in which someone had problems with many parts of the body and died as a result of movement of the intestines into the chest through an abnormal opening in the diaphragm. When this happens the intestinal material becomes trapped in the chest cavity and cut off from it's blood supply, leading to tissue death and edema. This would collapse the lungs.
