Multiple treatments: Fm is a quirky entity to treat. We really don't know what causes it and a lot of doctors "don't believe" in it. Find a good pain medicine or rheumatology doctor who is familiar with it and follow their advise. Exercise, warmth, decreasing stress and various medications such as cymbalta, lyrica, savella (milnacipran) and narcotics can help somewhat although none are "cures".

Sleep restoration: First restore seep sleep by removing impediments (e.g., narcotics), consider hypnotics (e.g., ambien). If those are ineffective, then consider electroprobe or laser or trigger point injection therapy or lyrica (pregabalin).
