Neither or both: RAD occurs due to RV pressure or volume overload which can be acute or chronic. Long term pressure overload will eventually lead to RVH. Long term volume overload will eventually lead to dilation. But you can have RAD acutely with neither. An example of that is acute pulmonary embolism.
