Reflux: Without other details, we can't be sure. He could be spitting up in his mouth. Another reason for the smell is post nasal drip: if he has nasal allergy and produces mucus in his nose, it could drip to the back of the throat and cause the breath to be foul. Take him to the doctor to have this sorted out.
