See a dentist: Does she brush 2-3 times a day, both gums and her tongue? Does she floss daily? She needs to have meticulous home care to reduce bacterial population. Is she seeing a dentist? There may be some underlying issue that's causing the malodor.

A visit to the: Dentist and pediatrician i'm assuming have already been done. Good hygiene (oral and overall) are obviously paramount. With that said there is a dental rinse called smart mouth available otc that I have had patients report really helps. Make sure the brushing of the teeth and tongue is happening at least 2x/day for at lest 2 minutes each time. A tongue brush system by tung is also very good.
