See ENT Specialist: He needs to be thoroughly examined by an ENT specialist to check for chronic sinusitis and gum disease. If he truly has "urine" like smell then he must have blood test done to check kidney function. Uremia causes such body odor.

See pediatrician: If your child has good oral hygiene and has not seen the doctor recently, get over there as soon as you can for a evaluation. Urine odor in the breath could mean that his/her kidneys are not working properly.
