NSAIDs & kidney: Non-steroidal anti-inflammatory medications such as ibuprofen are metabolized by the kidney. People with any abnormality in their kidney or with a history of kidney disease are often advised to avoid these types of medications due to small risks of kidney damage. Acetaminophen is a safer alternative for fever and pain relief.

Duplex kidney: I do not know of any problems of conservative treatment on Nurofen for pain or fever in a baby with duplex kidney. When you speak with your urologist, it would be a good idea to get a list of medications that should be avoided, especially if your baby's condition is associated with a ureterocele.
