Anything: Back pain is very non specific and be caused by multiple things such as muscle strains, bone disorders. In addition, internal issues such as lung problems, heart problems, and stomach/pancreas problems can also cause back pain. With the development of pain with respiration this should be evaluated urgently for things such as pulmonary emobli (blood clots).
