Varies: The dose of lovenox (enoxaparin) varies depending on whether it is being used for treatment 1mg/kg maximum 150mg/kg every 12hours, to prevent DVT(deep venous thrombosis) 40mg, which I think is what you are asking about and dose also changes for different surgical procedures and co-morbid conditions
