A new perspective: An evaluation from a different perspective might help provide the answer. An evaluation by either a physiatrist or by an orthopedist (one who deals particularly with foot diseases) seems to be indicated.

Spine?: It is possible that foot pain could be coming from a pinched nerve in the spinal area - have you had that checked? Sometimes the cause of pain just cannot be established - it is very difficult to deal with that especially when it's your foot! Good luck.
