A pulsation type of : Feeling may be vascular in origin but doesn't always have to be. A bad ache can feel like pulsation so can inflammation of a part of the body as it may be pressing close to a vessel. I would visit the foot doc and lets them give you an opinoin. They see heel pain everyday.

Monitor: One of the two arteries providing blood to the foot travels around the inside of the ankle bone possibly in the area that you describe.if it's not painful and doesn't change, it could probably just be followed and if it is the artery there is nothing that would need to be done anyway. This area is occasionally pinched by shoes that are tight around the top of the heel, so you could try other shoes
