Sounds like you have: an epigastric hernia. Anything that increases your intra abdominal pressure, including yelling, would cause pain. But hernia pain is usually located just around the hernia and would not radiate to other parts of the body. Get to a surgeon, to see if your symptoms are indeed coming from the hernia.
