MRI shows masses: MRI is designed to show only space-occupying lesions; Alzheimer's, unless in the advanced stages, results in brain changes, but does not alter the structure significantly enough to appear on MRI.

Alzheimer MRI: When a person has Alzheimers disease for a long time, there may be accelerated atrophy (shrinking) of the brain, inconsistent with their age, which may show up on MRI. At earlier stages of Alzheimers, the changes that are occurring happen within cells of the brain, and these are too small to be seen in a picture of the brain. The MRI is best for finding stroke, MS or other diseases.
