Varicocele seen by doctor about 5 years ago, sperm seemed fine and pain had subsided. lately i feel a dull ache. should i be concerned?

Yes: Varicoceles are not painful. So we are talking about something that is causing pain, this could be infection, injury, tumor, clot, hernia. You need to see whats going on which could be as easy as a limited exam in experienced office.
