Very Low Risk: Since the only data available on transmission from nipple to oral route of HIV transmission is derived from studies looking at breast-feeding babies with HIV+ moms the relative risk is approximately 1:3500 each day of exposure. As the child is receiving more fluid from the mother that simple nipple sucking would provide the risk an adult performing this act would encounter is likely even lower.

Only two situations: Where one could be exposed to HIV through sucking on a nipple. That would be if there is active bleeding from the nipple. Alternatively, if your female partner was lactating (making milk) after having a baby and not taking anti-HIV therapy. This scenario would be almost impossible unless your partner had not sought any medical care during pregnancy. Nipples that don't fit these situations are safe
