Similar but not same: Both Acitrom and Warfarin act in the same way, by blocking the effect of Vitamin K so you make less clotting factors. The main difference is how long the medication says in your body after taking a dose. Warfarin stays about 3 times longer. With both medications you need to check your blood test (INR or prothrombin time) regularly to make sure you are taking the right dose.
