Probably Bad GB: Why did you have the HIDA- I assume pain or nausea, bloating, dyspepsia after meals? Normal EF is 33-40% or more. If EF 20%, but not symptomatic and no other obvious cause, your likely problem is 'Biliary Dyskinesia' or that of a dysfunctional GB. Since no stones, no hurry on removal, let your symptoms dictate.

Yes: bile sludge and a new stone could have developed causing the problem and even inflammation or infection of the GB could occur causing the change in ejection fraction. If you are having symptoms with the change in EF you need to be seen. If no symptoms perhaps wait a few months and repeat the test
