Pain in upper back: Often these are pains from muscle spasms. There is a very thin muscles around the shoulder blade called rhomboid muscle and it has a tendency to go into spasm. However, if you have shortness of breath then you should see your doctor (and perhaps have a chest x-ray) to make sure you do not have spontaneous pneumothorax or more serious problems

See MD: It sounds like a musculoskeletal problem like a strain or sprain or inflammation of the posterior rib, but sharp back pain with inspiration can also be pleurisy which is inflammation of the outer lining of the lungs. Does the pain get better with over-the-counter pain medications? Please see your primary care physician if it gets worse, doesn't improve, you feel short of breath, develop fever.

Shoulder pain: I dont know how old you are, if you have had any strain or trauma, if you are experiencing shortness of breath. You may have a garden variety muscle pull or costochondritis, pleuritis or even spontaneous pneumothorax (tall, thin, white, young male). If its inflammation or a muscle pull, rest and Ibuprofen will help it, if its a dropped lung, you need to be seen in ER. You need to be seen by a doc.
