Extraction: Antibiotics won't cure the abscess but can alleviate some of the symptoms until definitive treatment such as extract of root canal can be done. Also antibiotics can reduce swelling. Sometimes the the swelling interferes with the local anesthetic. IV sedation will still work and treatment can be accomplished now. good luck.

Oral Surgeon: Your dentist might do best by referring you to an oral surgeon. The abscess may need to be drained, or you can be sedated to have the extraction performed. Seems like the tooth needs to come out, so the sooner the better.

3 Days....: Usually, swelling should get better or should not get worse after 3 days with antibiotic therapy. If it is not then I change antibiotic. I don't know how severe your swelling is. Why don't you have evaluated it with an oral surgeon? Extraction can be done under IV sedation.
