That's the hope: The 20 mg was a lower starting dose. If that did not completely control your symptoms after 4-6 weeks then it is appropriate to increase to 40 mg. the maximum dose is 80mg.

Therapeutic leve: The goal of the increased dose would be to decrease your psycological symptoms. Some people need higher doses to achieve their therapeutic goal.
