Filler best at first: I have to say I am somewhat biased against lip implants having seen other doctors patients with problems (infections or distortions) who want their implants removed. I would always start with a filler and see how the lips look make sure you are happy with the size and shape before jumping into something permanent. I would recommend fat grafting over implants - more natural/less risk of infection

The beauty is that these fillers are temporary, so if you don't like how it turns out you can either wait until it dissolves or actually have it reversed.Fat grafting is another alternative as well.
