What happens if i need pain reliever once a week for arthritis and i'm breastfeeding/pumping?

Some of that: medicine ends up in the breastmilk. although very small amounts it can be an issue if you start talking more than once a week. The baby can get slightly dependent on the medication and can cause withdrawal symptoms if its an opiate pain medication. If you are worried I would speak with your child's pediatrician.
