See doctor: Considering the symptoms you describe, it is possible you have an infection of your nailbed, which could be spreading - also known as "paronychia". You should see a doctor ASAP to get evaluated, as it may require antibiotics or drainage, and could get worse without it. In the meantime, apply warm soaks to your thumb at least 3x/day. If fever, go to ER. Best wishes.
