Mixed hyperparathy.: Hi. Secondary hyperparathyroidism (HPT) due to low vit. D NEVER causes hypercalcemia. By virtue of the fact you have hypercalcemia AND elevated PTH, you probably have primary HPT (need 24 hour urinary calcium to confirm; could be FBHH). The low vit. D adds a degree of secondary HPT, but secondary HPT CANNOT CAUSE your hypercalcemia. 3 months of calcium and vitamin D will give HIGHER hypercalcemia.

Treating vit d: I don’t think you have secondary HPT. With vitamin d replenishment, your calcium may increase. You need to look into primary HPT if the data you provided is correct.
