Mostly one suspects: it from the bone scans, XRays, or MRI/CT scans + the lab work indicates an infective picture. But most importantly it will be the presentation of the Pt, who will have acute pain with tenderness, fever, chills, etc. or obvious pus discharging. During surgery depending how long its been; pus maybe present, color changes in the bone, absence of bleeding from bone maybe seen.

Full thickness wound: If there is infected tissue down to bone, it should be considered infected bone. MRI is commonly used to determine the extent of an infection. If bone bleeds, it is viable. Bone biopsies, and bone cultures are methods to determine the extent of infection and viability of bone.
