Couple choices: A carotid duplex scan may be very useful in screening blood flow, but may get a bit more information with a CT-angio.

Yes probably: An ultrasound to check the carotids in the neck for stenosis. The carotid siphon is intracranial and won't be visualized by ultrasound. An MRA or CTA could visualize that region if needed.

Atrial Septal Defect: Atrial septal defects are usually diagnosed as infants, but many smaller versions are not. Atrial fibrillation and migraines are associated with these, but more typically migraines. Consider the next non-invasive test Echocardiogram with a Bubble study to establish left to right shunting in the heart.
