Streptococcus: The symptoms you present arre very suggestive of a streptococcal infection, probably tonsillar (which frequently may be felt in the ear (or radiate to the ear) by virtue of the location of tonsils. It is common in young children and can cause the symptoms you report. A good culture ( no rapid strep test: my experience has been poor in the past using such simple testing) appropriate antibiotic
