39 with sleep: All people have slow HR during sleep. 39 during sleep is harmless and not unusual. Pauses in the heart rate of >5 seconds generally require a pacemaker. Pauses in the HR that cause people to pass out (or nearly pass out) also require a pacemaker. HRs in the 30s during sleep never cause symptoms and don't require treatment.
