Severral things: Sometimes it could be a bladder problem tranmitting to the urethra, or could be other urethral disorders (stenosis, pain syndrome, diverticulum, etc) . You should get a pelvic exam and possibly a cystoscopy to see what is going on, provided all else is indeed normal.
