Can a hiatal hernia be missed or not picked up dying endoscopy? wouldn't a ct of abdomen show this?

Yes: It would depend on whether the hiatal hernia is up or down when the procedure is down. If the hiatal hernia is always above the diaphragm then it would show.

Yes: But ct no better. Any moderate hiatal hernia would be easy to note during endoscopy. Small intermittent hiatal hernias that are not visible during egd are likely of no clinical consequence. Best test may be impedance manometry, but to what end.
