Possible: Freiberg Disease is a painful collapse of the articular surface of the second metatarsal head. It may be associated either with or without trauma. It can occur at ages from 8-77, but is found more frequently in women then men. However, a 48 year old man certainly can have this condition.

A very rare Bird.: Understand that this "disease" is rare to begin with. It typically affects adolescent girls. It can affect boys , but a reduced (5:1) ratio. It generally affects the second or third metatarsal and the cause is thought to be vascular, traumatic and even genetic. A 48 year old man could have Freiberg's disease, but it would be considered extremely rare.

RARE: It would be interesting if this were looked at through an MRI and histologically,, to D/Dx from a normal variant and fracture, or unstable metatarsus w/ torn collateral ligaments. But, that makes it more expensive.
