Q-tip: Yes, or your Dr to get it out. Q-tips should not really be used to clean deep in your ears for that very reason. As the picture shows they are for cleaning what you can see, not what you can't. Anything deeper you should seek the care of a Dr or nurse who can see where they are placing the tip. And they even have special less dangerous mean of cleaning.

See doctor or ER to: get it out. Do not use Q-tips in the ear. Nothing smaller than your elbow in your ear.
