See your doctor: Grading cancers requires information about how big the tumor is, if there has been spread to other places in the body, and other factors. That being said, the symptoms you describe suggest that other things might be going on; your friend needs to see his oncologist straightaway, to discuss these symptoms and find out whether further tests are needed, or changes in his treatments.
